Nurse Practitioner; NP - East Mountains

Job Description

Corus Health is a full-service medical practice provider, seeking a qualified Nurse Practitioner who will be responsible for caring for patients, maintaining accurate and current patient records, and administering initial patient assessments and follow-up appointments as required. The NP will collaborate with our team of nurses and physicians to deliver premium care for our valued patients to assist with successful transitions through a variety of settings including hospitals, transitional care facilities, long-term care, assisted living, and private homes.

• Perform initial patient screenings, patient history, and physical examinations.
• Update patient records and cross-check records for accuracy at each appointment.
• Deliver patient-centered care according to clinical status, as well as patients’ personal goals and preferences.
• Perform hospice face-to-face assessments as required by Medicare & insurance to determine if patients qualify for services.
• Serve as a resource to patients for ongoing care information, counseling, and guidance for patients and their families.
• Assist patients and families with appropriate logistical arrangements.
• Work well and collaborate as an integral part of a team of nurses and physicians.

• Master’s Degree in Nursing
• 2+ years of applicable nursing and work experience, at least 1 year of home health experience preferred
• Valid Nurse Practitioner Licensure in the state of New Mexico
• Driver’s License and reliable, insured transportation
• Skilled in nursing practice, and able to cope with family emotional stress and tolerant of individual lifestyles
• Strong written and interpersonal communication skills
