Overview

Sub-Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (40 hours per week) Psychiatric Advanced Practice Provider for a float position among centers in Albuquerque, NM, that can be filled by a nurse practitioner or physician assistant. Prior APP experience in a psychiatric/mental health role is required.

Base salary range $120,000 – $130,000 plus a no‑cap incentive compensation bonus program.

Responsibilities
  • Reports directly to the Area Clinical Practice Manager and/or Director of Clinical Practice.
  • Collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities.
  • Collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.
  • Provides mental health primary and urgent care services to nursing center residents in collaboration with other medical providers.
  • Conducts psychiatric evaluations and assessments to determine mental health needs and performs ongoing medication management follow‑up appointments.
  • Performs psychotherapy for the treatment of mental health conditions.
  • Develops and implements treatment plans in collaboration with interdisciplinary care planning processes.
  • Prescribes and manages appropriate medications for mental health conditions.
  • Monitors and evaluates patient progress, adjusting treatment plans as necessary.
  • Completes all necessary documentation regarding patient health care in accordance with established policies, procedures and relevant State and Federal regulatory requirements.
  • Educates patients and their families about mental health conditions, treatment options, and self‑care strategies.
  • Follows evidence‑based practice guidelines.
  • Consults with a physician for areas outside the scope of practice.
  • Works with Align Med leadership team, center, and area market leadership to improve psychiatric quality measures.
  • Demonstrates continued professional competence by attending continuing education programs, maintaining certification, and applying standardized care guidelines.
  • Supports development of nursing staff in collaboration with the Center Nursing Executive, serving as a clinical expert and providing targeted clinical education.
  • Participates in center‑based quality initiatives, including preventing hospital transfer and improving residents’ mental health needs.
  • Ensures coding and billing compliance, timely submission of billable charges, and adherence to Align Med time‑off and on‑call policies.
  • Performs other duties as requested and supports Align Med clinical initiatives and compliance with legal and ethical standards.
Qualifications
  • Graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing; concentration/specialty track in Adult/Geriatrics and/or Family Nurse Practitioner (FNP).
  • Physician Assistant must be a graduate from an ARC‑PA accredited program, have completed the PA National Certifying Exam (PANCE) with PAC designation, hold state licensure and NCCPA certification.
  • Active certification as a Mental Health Nurse Practitioner or Physician Assistant.
  • Other concentrations/specialty tracks considered based on experience and business needs.
