Role:
Licensed Practical Nurse (LPN) in Long-Term Care (LTC)
Responsibilities:Provide direct patient and resident care according to individualized plans, safely administer medications and treatments within scope, monitor and document resident condition changes, assist with activities of daily living (ADLs), support assessments and care plan implementation, collaborate with healthcare team, and communicate compassionately with residents and families.
Qualifications & Skills:Active LPN license in New Mexico, experience in medication administration, assessments, ADLs, and documentation preferred, strong communication and teamwork skills, ability to work 12-hour day shifts, and reside locally in Albuquerque.
Key Duties:On-site care delivery in a skilled nursing facility, ensuring resident comfort, dignity, and high quality of life, and contributing to interdisciplinary team efforts.
