LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide high-quality nursing care to residents in long-term care or skilled nursing facility settings. Ensure patient safety, comfort, and adherence to clinical protocols.

Responsibilities:

• Administer medications and treatments accurately according to physicians’ orders.
• Monitor residents’ conditions and document changes or concerns promptly.
• Collaborate with nursing team to develop and update care plans.
• Maintain a sterile, safe, and organized environment during all clinical procedures.
• Comply with facility policies, infection control standards, and regulatory guidelines.

Qualifications:

• Valid LVN or LPN license recognized by the state.
• Proven experience in LTC or SNF environments preferred.
• Demonstrate strong assessment and clinical skills.
• Possess current CPR and first aid certifications.
• Ability to work effectively in a team and communicate clearly with residents and staff.
