LVN ​/ LPN – LTC​/SNF

Long-Term Care Nurse

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure residents receive high-quality, compassionate care aligned with healthcare standards and protocols.

Responsibilities:

• Administer medications and treatments according to physician orders.
• Monitor and document residents’ health status and changes.
• Coordinate with healthcare teams to develop and implement care plans.
• Educate residents and families about health conditions and prescribed treatments.
• Maintain accurate and complete patient records.
• Adhere to safety and infection control protocols at all times.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in a LTC or SNF environment preferred.
• Ability to follow detailed instructions and documentation procedures.
• Strong communication and interpersonal skills.
• Capable of working efficiently in a fast-paced healthcare setting.
• Compliance with COVID-19 vaccination requirements or approved exemptions.
