LVN ​/ LPN – LTC​/SNF

Nursing Assistant

Assist in providing quality nursing care to long-term care or skilled nursing facility residents under the supervision of registered nurses and physicians. Ensure patient safety, comfort, and adhere to facility policies at all times.

Responsibilities:

• Administer medication and treatments following established protocols.
• Monitor residents’ conditions and document any changes or concerns.
• Collaborate with healthcare team to develop and update care plans.
• Follow infection control procedures and maintain a safe environment.
• Communicate effectively with residents, families, and staff regarding care needs.
• Comply with all facility policies and regulatory requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of one-year experience in long-term or skilled nursing facilities.
• Ability to follow detailed instructions and adhere to safety protocols.
• Strong communication and interpersonal skills.
• Demonstrate ability to work effectively in a team environment.
• Current CPR certification required.
