LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse

Provide licensed practical nursing care in a skilled nursing facility, ensuring the well-being and safety of patients. Deliver compassionate support and maintain documentation of patient care activities.

Responsibilities:

• Administer medications and treatments accurately per physician orders.
• Monitor patient conditions and report changes to healthcare team.
• Assist with activities of daily living, including hygiene and mobility.
• Document patient interactions and care provided in compliance with facility protocols.
• Follow infection control procedures and safety guidelines at all times.

Qualifications:

• Hold a valid LVN/LPN license in the state of practice.
• Demonstrate previous experience in LTC or SNF settings.
• Complete a current COVID-19 vaccination or have a religious/medical exemption.
• Exhibit strong communication and patient care skills.
• Possess the ability to work independently and as part of a team.
