LVN ​/ LPN – LTC​/SNF

Long-Term Care/Skilled Nursing Facility Position

Assist in providing patient-centered care within a long-term care or skilled nursing facility setting. Ensure the safety, comfort, and well-being of residents while adhering to facility policies and healthcare standards.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor residents’ health status and report changes to the supervising nurse.
• Document clinical observations, interventions, and resident responses thoroughly.
• Assist residents with ADLs (Activities of Daily Living) to promote independence.
• Follow infection control procedures, including proper PPE use and documentation of COVID-19 vaccine status.
• Maintain a safe and clean environment in compliance with healthcare regulations.

Qualifications:

• Valid LVN or LPN licensure in the state of practice.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Must bring a valid state or federal issued  wear a photo badge at all times.
• Willingness to comply with vaccine requirements or approved exemptions.
• Ability to work effectively in a team and communicate clearly with residents and staff.
