LVN ​/ LPN – LTC​/SNF

LVN/LPN Position

We are seeking a qualified LVN/LPN to provide skilled nursing care in a long-term care or skilled nursing facility setting. The ideal candidate will ensure excellent patient care while adhering to all regulatory and safety standards.

Responsibilities:

• Administer medication and treatments to residents as prescribed by physicians.
• Monitor and document patient vital signs and condition changes.
• Communicate effectively with residents, families, and healthcare team members.
• Maintain accurate and detailed patient records in compliance with facility policies.
• Follow infection control procedures and ensure a safe environment.
• Support residents with daily activities and provide compassionate bedside care.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience working in LTC/SNF settings.
• Ability to efficiently perform clinical tasks and documentation.
• Strong communication and interpersonal skills.
• Uphold HIPAA regulations and patient confidentiality.
• Must have current COVID-19 vaccination documentation or an approved exemption.
