Long-Term Care/Skilled Nursing Facility Nurse

Provide skilled nursing care to patients in a long-term care or skilled nursing facility setting. Ensure delivery of high-quality patient care while adhering to facility protocols and safety standards.

Responsibilities:

• Administer medication and treatments as prescribed by healthcare providers.
• Monitor patient health status and document changes accurately.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Educate patients and families regarding health needs and post-discharge care.
• Maintain compliance with safety regulations and facility policies.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience working in LTC or SNF environments.
• Demonstrate strong communication and interpersonal skills.
• Exhibit excellent observation and critical thinking abilities.
• Complete required immunizations or valid exemptions.
