LVN ​/ LPN – LTC​/SNF

LVN/LPN Nursing Position

Provide quality nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Ensure patient safety, comfort, and adherence to facility policies and healthcare regulations.

• Administer medications and monitor patient vital signs.
• Document patient care activities accurately and promptly.
• Collaborate with healthcare teams to develop and implement care plans.
• Assist residents with activities of daily living.
• Follow infection control protocols and safety procedures.
• Communicate effectively with patients, families, and team members.

• Hold a valid LVN or LPN license in the state of practice.
• Demonstrate recent experience in LTC/SNF settings.
• Complete necessary vaccinations or have documented exemptions.
• Possess strong clinical assessment and documentation skills.
• Exhibit excellent communication and teamwork abilities.
