LVN ​/ LPN – LTC​/SNF

LVN/LPN Nursing Position

Provide quality nursing care in a long-term care or skilled nursing facility setting as an LVN/LPN. Ensure patient safety, comfort, and proper documentation in compliance with healthcare standards.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient vital signs and respond promptly to changes in condition.
• Document patient care activities thoroughly and maintain up-to-date records.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Follow facility policies and procedures, including safety and infection control protocols.
• Communicate effectively with patients, families, and staff regarding care needs.

Qualifications:

• License as an LVN or LPN in the state of practice.
• Proven experience working in a long-term care or skilled nursing facility.
• Strong understanding of nursing principles, procedures, and documentation.
• Ability to work efficiently in a fast-paced environment.
• Excellent communication and interpersonal skills.
• Must be able to wear a photo  and provide valid  vaccination documentation.
