LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide comprehensive nursing care to patients in long-term care or skilled nursing facilities, ensuring their safety, comfort, and well-being. Collaborate with healthcare teams to deliver high-quality patient services in a fast-paced environment.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient vital signs and observe for any changes in condition.
• Document patient care activities thoroughly and promptly.
• Communicate effectively with patients, families, and healthcare team members.
• Comply with all safety and infection control protocols, including proper identification and wearing of photo badges.
• Maintain current vaccination or exemption documentation as required by facility policy.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing settings.
• Ability to complete required documentation accurately and efficiently.
• Strong communication and interpersonal skills.
• Demonstrate adherence to safety, infection control, and facility policies.
• COVID-19 vaccination completion or valid exemption documentation.
