LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN)

Seeking a dedicated Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) for a local contract position in a long-term care or skilled nursing facility setting. This role involves providing quality nursing care to residents in a fast-paced environment.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor residents’ health status and report changes to the healthcare team.
• Document patient care activities clearly and promptly.
• Comply with facility policies, state regulations, and safety standards.
• Collaborate with multidisciplinary team members to develop and update care plans.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proof of current COVID-19 vaccination or valid religious/medical exemption.
• Prior experience working in LTC or SNF environments preferred.
• Ability to effectively communicate with residents and team members.
• Strong attention to detail and organizational skills.
