LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse

Provide licensed practical nursing care in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and well-being through professional nursing interventions and documentation.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient vital signs and overall condition.
• Assist with patient activities of daily living and personal hygiene.
• Maintain accurate and detailed patient records in compliance with regulatory standards.
• Collaborate with interdisciplinary team members to develop and implement care plans.

Qualifications:

• Hold an active and valid LVN or LPN license.
• Demonstrate minimum of 1 year of experience in LTC or SNF settings.
• Possess current CPR and IV certification if applicable.
• Exhibit strong clinical skills and attention to detail.
• Maintain compliance with state and federal healthcare regulations.
