LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

We are seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide quality patient care in a long-term care or skilled nursing facility setting. The ideal candidate will ensure patient comfort, safety, and well-being while adhering to facility policies and procedures.

• Administer direct patient care under the supervision of nursing staff and physicians.
• Monitor and document patient vital signs and symptoms accurately.
• Assist with patient mobility, hygiene, and comfort measures.
• Follow infection control protocols and maintain a safe environment for patients and staff.
• Communicate effectively with patients, families, and healthcare team members regarding patient status.
• Complete required documentation and compliance reports in a timely manner.

• Valid and current LVN or LPN licensure in the applicable state.
• Minimum of 1 year of experience in a long-term care or skilled nursing facility.
• Ability to bring a government-issued  wear a photo badge during shifts.
• Successful completion of the COVID-19 vaccination series or valid exemption documentation.
• Strong clinical skills and attention to detail.
