Luna Del Valle Hospice is proud to be 100% locally owned and operated, as well as nurse-owned and operated. Based in Albuquerque, we are passionate about expanding our services to rural areas of New Mexico to ensure access to comprehensive and compassionate end-of-life care. We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. • Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing care, including symptom and medication management.
• Coordinate resources, referrals, and care services for patients and families.
• Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning.

• Current licensure as a Registered Nurse (RN) in New Mexico.
• Experience in hospice or end-of-life care settings preferred.
• Strong critical thinking and assessment skills.
• Compassionate and empathetic approach to patient care.
• Excellent communication and teamwork skills.
