LVN ​/ LPN – LTC​/SNF

Job Summary:

Assist in providing high-quality patient care in a Long-Term Care or Skilled Nursing Facility setting. Perform delegated nursing duties under the supervision of registered nurses and medical staff to support patient recovery and well-being.

Responsibilities:

• Administer medications and treatments according to physicians' orders.
• Monitor patient vital signs and report changes to the healthcare team.
• Document patient care activities accurately and promptly.
• Assist patients with activities of daily living to promote comfort and independence.
• Adhere to facility policies and infection control protocols.
• Collaborate effectively with multidisciplinary team members to ensure optimal patient outcomes.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF environments preferred.
• Demonstrate strong clinical skills and attention to detail.
• Possess a current CPR certification.
• Ability to follow detailed instructions and work independently.
• Excellent communication and teamwork skills.
