CNA – LTC​/SNF

Job Summary:

Provide direct patient care in long-term care or skilled nursing facility settings, supporting patient comfort and safety under the supervision of nursing staff.

Responsibilities:

• Assist patients with activities of daily living, including bathing, dressing, and grooming.
• Monitor and report patient conditions, documenting vital signs and observed changes.
• Support mobility efforts, helping patients with transfers and positioning.
• Follow infection control protocols, including COVID-19 vaccination and safety measures.
• Maintain a clean and safe environment, respecting facility policies and procedures.

Qualifications:

• Hold a valid CNA certification or state license.
• Demonstrate completion of required COVID-19 vaccination or valid exemption.
• Possess excellent communication and interpersonal skills.
• Exhibit the ability to work collaboratively with healthcare teams.
• Show commitment to patient safety and quality care.
