LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide skilled nursing care to residents in a Long-Term Care/SNF facility as an LVN/LPN. Deliver compassionate patient care while supporting the healthcare team and maintaining compliance with facility and regulatory standards.

Responsibilities:

• Administer medication and treatments accurately according to physician orders.
• Monitor and document patient vital signs, symptoms, and progress.
• Assist with patient admissions, discharges, and care planning.
• Collaborate with interdisciplinary team to ensure high-quality resident care.
• Maintain healthcare records with precision and confidentiality.
• Follow infection control procedures and safety protocols diligently.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in a LTC or SNF setting preferred.
• Strong knowledge of medical terminology, nursing practices, and safety standards.
• Demonstrate excellent communication and interpersonal skills.
• Ability to work effectively in a team environment and handle multiple priorities.
