Hospice Clinical Liaison (RN)

Lily Care Hospice is a community-based hospice serving patients and families across the greater Albuquerque area. We are hiring a Hospice Clinical Liaison to help build our referral network from the ground floor and guide families to hospice at the right moment.

Duties and Responsibilities

  • Own the Albuquerque territory as our Hospice Clinical Liaison — build and grow referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices

  • Educate physicians, discharge planners, and case managers on hospice eligibility and the Medicare hospice benefit, and remove the barriers that keep patients from being referred in time

  • Review incoming referrals, obtain the H&P and clinical records needed to evaluate eligibility, and coordinate with the Clinical Director on complex cases

  • Meet with patients and families to discuss goals of care and explain what hospice at home actually looks like

  • Hand off admitted patients cleanly to the RN Case Manager and interdisciplinary team, track referral activity, and document accurately in the EMR

Compensation and Schedule

The Hospice Clinical Liaison role pays $90,000–$105,000 per year based on experience. Full-time, Monday–Friday days with flexibility to meet referral sources when they're available. Territory is the greater Albuquerque area, based out of our office at 4811 Hardware Dr NE, Albuquerque, NM.

Skills and Qualifications

  • Current RN license in New Mexico or a compact state

  • Two years of RN experience; hospice, home health, or palliative care experience strongly preferred

  • Existing relationships with Albuquerque-area referral sources are a significant advantage

  • Comfort initiating conversations with physicians, discharge planners, and families about end-of-life care

  • Valid driver's license and reliable transportation
