LVN​/LPN - LTC​/SNF

Licensed Vocational Nurse (LVN/LPN)

Seeking a licensed vocational nurse (LVN/LPN) for a six-week contract to provide quality patient care in long-term care or skilled nursing facilities. The role requires adherence to facility policies and an ability to work efficiently in a fast-paced environment.

Responsibilities:

• Provide direct nursing care to patients in LTC/SNF settings.
• Administer medications and treatments as prescribed by healthcare providers.
• Document patient assessments, interventions, and outcomes accurately.
• Communicate effectively with patients, families, and healthcare team members.
• Adhere to all facility policies, safety standards, and infection control protocols.
• Maintain compliance with vaccination and identification requirements, including bringing a valid  necessary immunizations.

Qualifications:

• Valid state license as an LVN or LPN.
• Proven experience in LTC or SNF environments preferred.
• Ability to follow detailed instructions and ensure patient safety.
• Strong communication and interpersonal skills.
• Must have completed COVID-19 vaccination or possess an approved medical/religious exemption.
• Ability to work flexible shifts including weekends.
