UNM Hospital Sandia Vista Clinic is looking for a motivated clinician who is interested in joining our team to help care for patients in our local community. The newly built clinic is located in Southwest Albuquerque. The Family Medicine practice focuses on seeing patients of all ages with acute and chronic medical and behavioral health related issues. Working together as a multi-disciplinary team that consists of MDs, Residents, PAs, NPs, Psychologists, Clinical Pharmacists, Care Managers, Social Workers, and Community Health Workers allows us to provide an overall comprehensive care approach.
With our Patient Centered Medicine Home (PCMH) clinic model, providers collaborate with families to identify and satisfy the full range of patient needs and preferences. Providers work semi-autonomously with collegial support and resources that are available. This is an exciting career prospect, which offers many opportunities for development of new skills, growth, collaborative teamwork, learning, and teaching while contributing to our commitment to providing quality care for patients and the community.

Position Summary:Function in a primary care setting in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care

Ages of patients served may include neonate, pediatric, adolescent, adult and geriatric age groups. Senior Health Clinic:

- PATIENT CENTERED MED
- Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable - PATIENT CARE
- Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute - PATIENT CARE
- Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved health - PATIENT CARE
- Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment - DOCUMENTATION
- Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records - PATIENT CARE
- Perform invasive and non-invasive diagnostic and therapeutic procedures - ROLE MODEL
