CNA - LTC​/SNF

Job Summary:

We are seeking a dedicated CNA to join our healthcare team, providing essential support to patients in long-term care or skilled nursing facilities. The ideal candidate will demonstrate compassion, attention to detail, and a commitment to quality patient care.

Responsibilities:

• Assist patients with activities of daily living, including bathing, dressing, and mobility.
• Monitor and document patient vital signs and condition changes.
• Follow care plans accurately and report any concerns to nursing staff.
• Maintain a clean and safe environment for patients and staff.
• Comply with all facility policies and infection control procedures.
• Assist with repositioning and transfers to prevent pressure ulcers and injuries.

Qualifications:

• Valid CNA certification in the state of practice.
• Proven experience working in LTC or SNF settings preferred.
• Ability to handle physically demanding tasks and lift patients safely.
• Strong communication skills and compassionate demeanor.
• Ability to work flexible shifts, including weekends and holidays.
• Successful completion of required background checks and health screenings.
