Job Summary:

Provide direct patient care and support within a Long-Term Care/SNF setting, ensuring residents' needs are met with compassion and professionalism. Collaborate with healthcare teams to promote a safe, comfortable environment for all residents.

Responsibilities:

• Assist residents with activities of daily living, including bathing, dressing, and feeding.
• Monitor and document residents' conditions, reporting any changes to nursing staff.
• Follow infection control protocols and maintain a clean, safe environment.
• Support medication administration under supervision, ensuring proper hygiene and safety procedures.
• Respond promptly to residents' calls and emergencies with appropriate action.
• Promote resident well-being through compassionate, respectful care.

Qualifications:

• Valid CNA certification/licensure.
• Proven experience in LTC/SNF environments preferred.
• Ability to follow detailed care plans and instructions.
• Strong communication and interpersonal skills.
• Commitment to patient safety and infection control standards.
• Must meet facility-specific vaccination and identification requirements.
