CNA – LTC​/SNF

Job Summary:

Provide direct patient care to residents in long-term care and skilled nursing facilities, ensuring their safety, comfort, and well-being. Assist nursing staff with daily activities and uphold high standards of patient care.

Responsibilities:

• Perform ADLs such as bathing, dressing, and toileting to support resident independence.
• Monitor and document patient vital signs and behaviors accurately.
• Assist with mobility and transfers, ensuring safety protocols are followed.
• Support nursing staff with patient care tasks and reports any concerns or changes.
• Maintain a clean and safe environment, adhering to facility policies.
• Follow state and federal identification and vaccination requirements for all staff members.
  
  Qualifications:
• Valid CNA certification in the relevant state.
• Proven experience working with elderly or long-term care residents.
• Ability to perform physical tasks such as lifting and transferring patients.
• Strong communication skills and attention to detail.
• Must provide valid photo identification and proof of COVID-19 vaccination or exemption.
• Comply with facility policies regarding identification and vaccination mandates.
