Job Summary:

Provide high-quality Licensed Vocational Nurse (LVN) / LPN care in a long-term care and skilled nursing facility setting. Deliver direct patient care, support clinical operations, and ensure compliance with healthcare standards.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely.
• Monitor and document patient health status and report significant changes.
• Assist with patient hygiene, dressing, and mobility needs.
• Follow infection control protocols and maintain a safe environment.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Ensure compliance with state and federal healthcare regulations.

Qualifications:

• Valid LVN/LPN license in the state of New Mexico.
• Prior experience in long-term care (LTC) and post-acute care (PCC) settings.
• Ability to work independently with minimal supervision.
• Demonstrate strong clinical skills and attention to detail.
• Ability to adapt quickly in a fast-paced environment.
• Proven competency in patient assessment and care planning.
