LVN​/LPN – LTC​/SNF

Job Summary:

Seek a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide patient care in a long-term care or skilled nursing facility setting. This role involves delivering high-quality nursing support, monitoring patient conditions, and collaborating with the healthcare team to ensure optimal patient outcomes.

Responsibilities:

• Administer basic nursing care procedures, including medication administration and vital sign monitoring.
• Document patient observations and update care records accurately.
• Educate patients and families on health management and treatment plans.
• Collaborate with interdisciplinary team members to develop and implement patient care plans.
• Comply with infection control policies, including adherence to vaccination and PPE protocols.
• Maintain a safe and hygienic environment, following all facility guidelines.

Qualifications:

• Valid LVN/LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Certification in CPR and basic Life Support (BLS).
• Ability to properly use medical equipment and monitor patient status.
• Excellent communication and interpersonal skills.
• Must provide a valid  have completed COVID-19 vaccination or approved exemption.
