LVN​/LPN – LTC​/SNF

Job Summary:

Seeking an experienced LVN/LPN with a strong background in long-term care (LTC) and personal care services (PCC) to provide quality patient care in a skilled nursing facility setting. The ideal candidate must be ready to start immediately with minimal orientation.

Responsibilities:

• Administer nursing care and medication to patients in accordance with provider plans.
• Monitor patient conditions and report changes to healthcare teams promptly.
• Document patient assessments, treatments, and responses accurately.
• Follow all safety protocols and infection control procedures.
• Collaborate with multidisciplinary team members to ensure excellent patient outcomes.
• Assist with patient hygiene, mobility, and daily living activities as needed.

Qualifications:

• Valid LVN/LPN license recognized in the state of practice.
• Prior experience in LTC and PCC settings required.
• Ability to work independently and hit the ground running.
• Excellent assessment, communication, and documentation skills.
• Must have up-to-date COVID-19 vaccination or valid exemption documentation.
• Reliable transportation and ability to work on a flexible schedule.
