fostering connections provides services and supports for young people from ages 16.5-26 to include assistance with job applications, college applications, support with medicaid and snap and any other services needed while enrolled in fostering connections. Fostering connections assists with different funding sources to assist with educational training and funding to support with furniture, deposits for housing and other resources to assist young people to transition successfully into adulthood.

* why does the job exist?
this position exists to utilize data to determine compliance with federal and state regulations and requirements (e.g. Chafee, etv, families first, etc.) related to older youth services. The incumbent coordinates communication and reporting regarding federal compliance in relationship to older youth services with internal partners and state/federal reporting entities. This position provides technical assistance to oys staff, internal partners and others regarding federal requirements and efforts towards 100% compliance in providing services and supports for youth transitioning from foster care to adulthood;
the incumbent will partner with public/private agencies that serve transition-aged youth and receive federal pass through funding (iv-e/chafee) to strengthen these relationships and improve access to services and supports.
how does it get done?
the ideal candidate has experience as it relates to:
high school diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care.
