Leo Force is seeking a dedicated Case Manager – Community Health for our Albuquerque-area FQHC. You will coordinate care, connect patients to community resources, and work with medical, behavioral health, and social services teams to improve overall health outcomes.
You will advocate for patients, addressing social determinants of health such as housing, transportation, food access, and behavioral health support. This onsite role runs Monday–Friday, 8:00am–5:00pm.
