Social Worker
DutiesThe social worker is assigned to the Primary Care-PACT. The goal is to support veterans' stability and well-being through comprehensive, collaborative interventions. They must expertly assess and treat Veterans' complex psychosocial issues affecting health and care. Responsibilities include evaluating stressors such as medical adjustments, mental health or substance abuse, economic hardship, legal problems, and housing or transportation barriers. Referrals come from the interdisciplinary team, Veterans, families, or community professionals.
The incumbent conducts thorough assessments, identifies root causes, and promotes understanding of contributing factors. They collaborate with patients and families to explore treatment options, encouraging lasting positive changes. Coordination with VA staff and community agencies is essential to problem-solving and service delivery. Case management involves working with the outpatient team to facilitate access to services like transportation, advance directives, financial aid, housing, and resources to address functional decline.

Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
