The incumbent oversees all aspects of HUD-VASH case management for assigned Veterans, including:
- Verifying Veteran eligibility for VA medical care and program participation.
- Assisting Veterans with voucher paperwork and supporting them during the voucher process.
- Using Housing First and evidence-based models to foster engagement and self-efficacy.
- Assisting with housing placement from voucher acquisition to lease-up.
- Advocating for Veterans with landlords, PHA, community providers, and VA stakeholders.
- Conducting assessments and developing treatment plans.
- Helping Veterans set and achieve goals related to employment, healthcare, social supports, benefits, and end-of-life planning.
- Coordinating care by linking Veterans to VHA services, VA offices, and community agencies.
- Managing a caseload from initial assessment through long-term housing stability.
- Providing intensive case management for ongoing needs to maintain housing and treatment goals.
- Delivering social work services in individual or group settings in accordance with VHA guidelines.
- Participating in program calls, training, and adhering to VA policies, including data collection and reporting for program accreditation.
- Contributing to program planning and policy development based on caseload trends and work experience.
- Pursuing ongoing professional development through seminars and training.
- Documenting all contacts, completing reports, and updating the Homeless Operations Management and Evaluation System (HOMES).
- Collaborating with NMVACHS staff and community agencies to improve healthcare delivery for HUD-VASH Veterans.
- Practicing within the ethical guidelines of the NASW Code of Ethics and NMVAHCS policies.

Monday through Friday, 07:30 a.m.

Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement:
