Case Management + Care Coordination | CPSW / CSW / CCSS Backgrounds | Up to $20/hr A bit about us:A federally qualified health center in Albuquerque is seeking a Case Manager to support individuals and families through case management, care coordination and connection to community-based services.
The organization provides healthcare and support services to underserved populations, including individuals experiencing homelessness and patients with complex medical, behavioral health and social-service needs. The Case Manager will work directly with patients and interdisciplinary teams to identify barriers to care, coordinate services and connect individuals with appropriate internal and community resources.
Experience in community outreach, housing programs, public health, harm services, HIV services, peer support or community-based case management is especially relevant to the work.

Job DetailsThe Case Manager will provide case management and care coordination services for individuals and families receiving services through the health center.
- Provide ongoing case management and care coordination for an assigned caseload
- Assess patient needs and identify medical, behavioral health and social barriers to care
- Connect patients with community resources and support programs
- Coordinate referrals for housing, transportation, food assistance, behavioral health and other social services
- Assist patients with navigating healthcare and community-based systems
- Develop and maintain individualized service or care plans
- Follow up on referrals and services to help maintain continuity of care
- Advocate for patients and help address barriers that may prevent access to needed services
- Work with individuals and families from underserved and vulnerable populations
- Collaborate with medical, behavioral health and social-service teams
- Coordinate with outside agencies and community organizations when additional services are needed
- Maintain accurate and timely case management documentation
- Document assessments, referrals, patient interactions and follow-up activity in the electronic health record
- Track patient progress and update service plans as needs change
- Support patient engagement and education around available services and resources
- Participate in community outreach and related support activities as needed
- Previous case management, care coordination, peer support or community-service experience
- Experience working with individuals, families and underserved populations
- Certified Peer Support Worker (CPSW) preferred
- Bachelor’s degree preferred
- Backgrounds in Peer Support, CSW, CCSS, community health, behavioral health or social services are encouraged
- Experience with community outreach, housing programs, public health, harm services or HIV services preferred
- Familiarity with community resources and social-service programs
- Strong case documentation and follow-up skills
- Ability to coordinate services across multiple providers and community organizations
- Experience with electronic health records or case-management documentation preferred
- Bilingual skills are a plus
- Must successfully complete required fingerprint/background screening
- Must provide required vaccination documentation or approved exemption
- Must successfully complete pre-employment drug screening
