Adelante is seeking a Service Coordinator to support clients and families by coordinating services and monitoring therapy plans. You will work with Adelante staff, guardians, and case managers to ensure individual service plans meet client needs and advocate for services.
You will manage crisis interventions, track ISP progress, and coordinate inter-agency efforts, while maintaining accurate reporting and funding for clients.
