Nurse Practitioner – Infusion Services (Christus Health, Alexandria, LA).
OverviewUnder the supervision of the School Based Health Centers Director, the Nurse Practitioner is responsible for providing primary care to clinic patients in accordance with the Louisiana Nurse Practice Act. The practice remains within the scope delineated by education and training. The NP delivers care and treatment, including routine care to adolescents and children, following health center policies and protocols. The NP participates in the development, execution, and periodic review of written policies governing the services offered by the health center.
In collaboration with the Medical Director, the NP reviews patient health records and arranges referrals for services that cannot be provided at the center. The NP ensures adequate patient health records are maintained and transmitted appropriately when patients are referred to other providers.
- Provides primary health care in a school setting.
- Elicits pertinent medical and psycho‑social history including patient profile, chief complaint, and history of present illness, past medical history, social history and review of body systems.
- Performs a physical examination as pertinent, modifying the examination for the nature of the complaint or problem, age/sex, and physical/mental condition of the student.
- Orders and/or performs laboratory, X‑ray and diagnostic studies appropriate for complaint, age, race, sex and physical condition of the student.
- Documents in the medical record according to policy and procedure.
- Allows for privacy and modesty in the provision of care.
- Completes medical evaluation forms including sports physicals and medical home physicals on qualifying students.
- Distinguishes between normal and abnormal information obtained from history, physical and screening examination of adolescents and pediatric patients.
- Develops preliminary interpretation of diagnostic test and other data results with impression as to severity, etiology and immediacy of the patient’s problem.
- Develops a plan of care for students with follow‑up and referrals as needed.
- Applies established therapeutic practices within the scope of practice, demonstrating sound clinical judgement in decision‑making.
- Administers or prescribes medications as needed.
- Initiates appropriate management for emergency situations until EMS is available.
- Provides education/training to patient and family members.
- Develops a health management plan considering patient’s problem, age, sex, race, and condition, and involves the patient’s family as appropriate.
- Identifies and documents patient/family educational needs upon initial assessment.
- Identifies barriers to learning.
- Provides teaching based on identified needs (cultural, ethnic factors, functional).
- Evaluates the effectiveness of instruction provided.
- Advocates on behalf of students concerning physical health issues between parent, physician, and community.
- Demonstrates knowledge of NP‑specific policies and procedures and the ability to safely and competently perform quality patient care.
- Maintains continuing education as needed for licensure.
- Attends hospital‑wide and school‑based health in‑services to maintain and increase knowledge of related issues.
- Demonstrates knowledge of policies, manuals, and chain of communications.
- Obtains necessary training prior to initial procedure or equipment use.
- Ensures equipment is in operating order prior to use, adhering to operating guidelines and differentiates between patient complications and equipment malfunction.
- Provides supervision for RN in the center.
- Conducts chart review of RN, maintains scheduling and forms.
- Conducts annual department competencies checklist with RN.
- Orients RN to new skills.
- Provides care in the health center when RN is absent.
- Is available to RN by telephone for consultation when not on site.
- Master of Science in Nursing.
- Minimum one year of experience preferred.
- APRN License in the state of employment.
- RN License in the state of employment or compact.
- BLS certification.
5 Days – 8 Hours per day.
Full Time.
