Salary: $29-$35/hr based on licensure and experience
Schedule: Day shift

- Conduct and coordinate comprehensive assessments of new and existing residents using the MDS tool.
- Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
- Ensure compliance with federal and state regulations for MDS assessments and care planning.
- Monitor and transmit MDS assessments to the relevant authorities.
- Train and guide nursing staff on MDS procedures and updates.
- Audit clinical records to ensure accuracy, completeness, and regulatory compliance.
- Collaborate with healthcare providers to optimize resident care outcomes.
- Active RN license required.
- MDS Certification preferred.
- Proven experience in clinical assessment and MDS coordination.
- Strong knowledge of federal and state regulations related to MDS and care planning.
- Exceptional organizational skills and attention to detail.
- Excellent communication and interpersonal skills to interact with staff and residents.
- Oversee and manage clinical operations of the assigned unit.
- Coordinate with interdisciplinary teams to ensure delivery of high-quality care.
- Mentor, guide, and supervise nursing staff, ensuring adherence to policies and standards.
- Lead care planning meetings, evaluating residents' needs and establishing care priorities.
- Address and resolve any patient or family concerns in a timely manner.
- Monitor performance metrics, striving for continuous quality improvement in the unit.
- Collaborate with other unit managers and facility leadership to optimize overall patient care and operations.
- Current Registered Nurse (RN) certification in State of Louisiana
- Experience in leadership roles within a healthcare setting.
- Strong clinical, organizational, and managerial skills.
- Exceptional communication abilities and a focus on team collaboration.
- Passion for ensuring the highest standards of care and resident satisfaction.
