Registered Nurse, Home Health

• Ensure activities align with Elara Caring’s vision and Home Health leadership
• Maintain patient and staff privacy and confidentiality under HIPAA
• Assess assigned caseloads of home health patients and families
• Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating treatment plans
• Assess physical, psychological, social, and spiritual factors affecting patient and caregiver health
• Conduct initial evaluations and re-evaluate nursing needs during visits
• Make follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, health changes, environmental concerns, and unsafe facility conditions to relevant personnel
• Revise plans of care in response to patient care issues and notify clinical leadership, physicians, facilities, and team members
• Report to the Clinical Manager or Clinical Supervisor

• Current, unrestricted state RN license
• 1+ year of experience in a clinical care setting as a nurse
• Ability to frequently lift, push, pull, and support up to 50 pounds
• Ability and willingness to travel within the branch/office coverage area
• Dependable vehicle
• Valid driver’s license
• Current auto insurance in accordance with state laws
• Ability and willingness to travel 50%
• Associate’s Degree or Bachelor’s Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Previous experience with Home Care Home Base preferred
• Ability to maintain HIPAA privacy and confidentiality

Demonstrates expertise in patient assessment, care planning, and coordination in home health settings, with a strong emphasis on maintaining HIPAA compliance and effective communication with clinical teams. Holds a current RN license and possesses the ability to manage caseloads while ensuring patient and caregiver health.
