Candidates must have case management experience. Candidates must have experience using Midas and Inter Qual. Candidates must hold a licensed master's degree.
Description:
Typical census is 180-230 patients. There are a total of 6 case managers in the unit. There is 1 case manager weekend option available covering Saturday and Sunday from 7 AM to 7 PM. Four to five case managers work Monday through Friday from 8 AM to 4:30 PM. Patient ratios are 1:40 during weekdays. Equipment used includes laptops and cell phones.
The electronic medical record system utilized is Medi Tech and Midas. Candidates with knowledge of area resources are preferred. Responsibilities include interviewing patients and families face-to-face. Daily multidisciplinary rounds (MDRs) are conducted on each unit. Inter Qual reviews are performed on every patient. Observation reviews, discharge planning evaluations (DPEs), and discharge planning are also key tasks. The ideal candidate should be personable and able to work well with others.
Floating may be required to various departments within the facility. The dress code includes Caribbean blue scrubs and a white jacket. Rotating weekends are required, specifically Saturday and Sunday from 8 AM to 4:30 PM. No on-call duties are required.
