Diversion Case Manager

Eckerd Connects seeks a Diversion Case Manager to support youth and families at risk of deeper juvenile justice involvement. In this community-based role, you will complete intake and risk/needs assessments, develop individualized diversion plans, and connect clients to counseling, education, workforce, and family support services. You will coordinate with courts, probation, schools, and providers, track progress, and maintain detailed documentation.
