Purpose and Scope
The professional registered nurse Home Therapies RN CAP 2 is an entry level designation within the Clinical Advancement Program (CAP) for new employees who meet RN CAP 2 criteria or gain it through advancement from RN CAP 1. This position is accountable for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients.
It may involve delegation of tasks to RNs, LVNs/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, the nurse participates in decision‑making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
- Perform all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor, or another registered nurse.
- Collect and analyze dialysis data for assigned patients, document findings, adjust treatment plans as needed, and notify the Supervisor or physician.
- Assess, collaborate with, and document patient/family learning needs to provide initial and ongoing education.
- Direct and provide, in collaboration with the patient, home care partner, and ancillary staff, all aspects of safe and effective dialysis delivery.
- Administer medications as prescribed or according to approved algorithms, documenting justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
- Ensure correct laboratory collection, processing, and shipping procedures are performed, and reschedule missed or insufficient collections.
- Identify expected outcomes and update the nursing assessment and plan of care through collaboration with the Interdisciplinary Team.
- Ensure patient awareness of transplant and treatment modality options.
- Assist with identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners.
- Assess barriers in the candidate’s home environment and partner/family readiness for home dialysis.
- Train home dialysis patients and/or Home Partners on the safe operation and maintenance of equipment and supplies through a standardized training program.
- Participate in education and quality improvement projects at the facility and area level, as directed by the Supervisor.
- May serve as a Preceptor to new employees.
- Complete CAP requirements to maintain or advance the position.
- Perform all other duties assigned by the Supervisor.
- Day‑to‑day work includes desk work, computer work, interaction with patients, facility/hospital staff, and physicians.
The role involves frequent, prolonged standing, bending, lifting patients, and moving equipment up to 200 lbs. assistance from coworkers or aids is allowed. The position requires heavy lifting, patient assistance, and the ability to lift chemical and water solutions up to 30 lbs. as high as 5 feet. The work environment is a health care facility with controlled air temperature and moderate noise.
Exposure to infectious or contagious diseases/materials may occur. The nurse may rotate coverage with other licensed home therapy staff, participate in on‑call rotations, night, weekend, holiday schedules, and travel to training sites or patient homes.
The RN is assigned oversight of RNs, LPNs/LVNs, Patient Care Technicians, and Home Therapies Care Team Assistants as a Team Leader or designated Nurse in Charge, after meeting all required education and training and having at least three months of home dialysis experience as an RN.
Education and LicensureGraduate of an accredited School of Nursing, current state licensure, and current or successful completion of CPR/Basic Life Support certification. Must meet the practice requirements in the state of employment.
Experience and Required SkillsEntry level for RNs with a minimum of two years of nephrology nursing experience within the last two years as an RN. Must be able to work independently, communicate effectively, and demonstrate clinical competence in home dialysis therapies.
