Constellation Health Services is seeking a Nurse Practitioner to deliver comprehensive hospice care under physician supervision in patients' homes and communities in Virginia. You will assess, diagnose and develop plans of care, coordinating with the hospice team.
You will perform face-to-face visits, monitor symptoms, adjust treatments, and educate families while ensuring compliance with state nurse practice acts and HIPAA requirements. Travel within the service area is expected.
