Part-Time Rn Wellness Nurse (Evening Shift)
This position is primarily responsible for monitoring the clinical and holistic health of assisted living residents, coordinating necessary services, and delivering care utilizing the nursing process of assessment, planning, implementation, and evaluation, while maintaining the standards of professional nursing. The wellness nurse provides person-centered care in collaboration with care partners, medication aides, and the interdisciplinary team.
Key duties include assigning tasks to care partners and/or med techs in accordance with care plans and individual service plans, functioning as a clinical resource person, coordinating communication with the provider and other ancillary services to direct resident care on a designated shift, conducting daily wellness checks to monitor, evaluate, and treat residents who have had a recent or ongoing change in condition, coordinating resident medical appointments and transportation as requested, and providing leadership by being a role model and providing the highest quality service to customers including residents, internal customers, and partners/suppliers.
Job requirements include being a graduate of an approved RN program, having current licensure in the Commonwealth of Virginia or compact state of residence, having a minimum of two years of nursing experience, having current CPR certification, and possessing the ability to deal tactfully with residents, staff, and family members.
