Full-Time RN​/LPN Wellness Nurse (Day Shift

Schedule:

Day shift (7 a.m. to 3 p.m.)

Pay:
Competitive pay for RN Wellness Nurses ranging from $38 to $52 per hour based on experience and credentials.

This position is primarily responsible for monitoring the clinical and holistic health of Assisted Living residents, coordinating necessary services, serving as the infection control nurse, providing clinical training and delivering care utilizing the nursing process of assessment, planning, implementation and evaluation, while maintaining the standards of professional nursing. The Wellness Nurse provides person‑centered care in collaboration with care partners, medication aides and the interdisciplinary team.

• Assign tasks to Care Partners and/or Med Techs in accordance with care plans and individual service plans.
• Act as a clinical resource person coordinating communication with the provider and other ancillary services to direct resident care on a designated shift.
• Conduct daily wellness checks to monitor, evaluate and treat residents who have had a recent or ongoing change in condition, including collaboration with interdisciplinary team members.
• Coordinate resident medical appointments and transportation as requested.
• Provide leadership by being a role model and delivering the highest quality service to customers including residents, internal customers, and partners/suppliers.

• Graduate of an approved RN program.
• Current licensure in the Commonwealth of Virginia or a compact state of residence.
• Prefer a minimum of two years of nursing experience.
• Current CPR certification.
• Ability to deal tactfully with residents, staff and family members.
