We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Algonquin area. This position is eligible for a total bonus potential of $20,000, including a $10,000 first‑year success bonus to reward the accomplishment of key achievements during your first year, as well as an additional $10,000 sign‑on bonus paid within the first 18 months of employment.

- Deliver skilled, patient‑centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
- Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
- Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.

- Active Physical Therapist, PT license in the state you work.
- One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
- Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.

Includes compensation related to patient care visits, non‑visit activities, paid days off, shift premium pay, and first‑year success bonus. Low end of the range is the average annual compensation earned based on a fully productive caseload. The high end of the range is reflective of those who exceed productivity.
