Home Health Care | Full-Time

Provides Physical Therapy services to patients in their homes, assessing functional levels and developing treatment programs to improve independence and physical/cognitive functioning. This role includes patient and caregiver education, supervision of support staff, and ongoing progress reporting.

  Master's degree or higher
• Experience:
  
  Healthcare Setting (1+ years)
• Licenses/Certifications:
  
  Valid Physical Therapist license in the state of Illinois (IL) Valid Regular Driver's License (DL)
• Preferred: PT license number (to be provided during application)

Monday – Friday, 8: 00a – 5:00p Plus one weekend per month and 2 holidays per year Typically sees 5–6 patients per day

Travel:
Local travel to residential homes required

• Perform Physical Therapy assessments and develop individualized treatment programs
• Record and report patient's reaction to treatment and changes in condition
• Instruct patients and caregivers in therapy programs and proper device use
• Supervise physical therapy assistants and, when appropriate, certified nursing assistants
• Prepare reports and participate in staff and Agency development activities
