Job OverviewWe are looking for a full‑time home health registered nurse case manager who will lead and collaborate to provide management and delivery of patient care plans for patients in Algonquin, IL and surrounding areas. The role comes with a $10,000 first‑year success bonus for achieving key achievements in your first year.
Responsibilities- Provide patient care in the home setting following established care plans.
- Collaborate with interdisciplinary teams to manage and deliver patient care plans.
- Drive to and from patient locations, spending approximately 20% of your day on travel.
- Maintain accurate and timely charting to meet critical deadlines.
- Self‑manage time and schedule to meet care needs.
- RN license in the state you work.
- Graduate from an approved school of practical nursing.
- At least one year of RN experience and clinical competence to deliver quality patient care.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- Commitment to consistently meet critical charting deadlines.
- Strong time‑management and self‑management skills.
- Demonstrated experience with tablets, mobile phones, and EMR software.
