Every service is delivered at no cost, regardless of insurance status.

As QA Nurse / Compliance Coordinator, you will own clinical quality, regulatory compliance, and survey readiness for our home health program. You will conduct chart audits, drive performance-improvement initiatives, and ensure we are always survey-ready.

• Conduct OASIS chart audits and clinical-record reviews to identify quality opportunities
• Lead performance-improvement projects targeting specific Star Ratings, HHCAHPS, and outcome metrics
• Prepare the agency for state and CMS surveys; manage corrective-action plans when needed
• Train clinical staff on documentation standards, infection control, and patient safety
• Track and report on quality indicators monthly to the leadership team
• Investigate complaints and adverse events; document findings and follow-up

• Current CA RN license + BSN preferred
• 3+ years home health nursing with at least 1 year in QA or QAPI
• OASIS-C certification (COS-C) strongly preferred
• Familiarity with CMS Conditions of Participation, Star Ratings, HHCAHPS
• Strong attention to detail and data-analysis skills
