Field LVN

First Home Healthcare is a locally owned agency that has been in business for over 10 years. We are a 4.5‑star agency and we are currently looking for a reliable, compassionate LVN to care for patients in their home. Must be professional and have home‑health experience. You will work under the supervision of a registered nurse. We offer competitive pay and benefits.

• Must have a good standing LVN license in the state in which the clinician will practice.
• Must have at least 6 months of hands‑on nursing experience.
• Current CPR certification.
• Must be willing to travel & provide in‑home nursing care to adult and/or geriatric patients.
• Home‑health experience highly preferred.

• One‑to‑one patient care in a home environment.
• Execution of physician‑prescribed plan of care and compliant documentation of care provided in the system of record.
• Administration of prescribed medication, treatments, and therapies.
• Patient assessments and coordination of care.
• Health promotion, teaching, and training of patient and family members.
