Provides Early Childhood Intervention (ECI) services for children (ages 0-3) and their families as identified in the Individual Family Service Plan (IFSP). Services include screening, intake, assessment, referral, planning, data collection, special instruction for child and family service coordination. Travels in assigned area to deliver services.
Primary Responsibilities- Assists in screening, assessment and referral services to eligible children and their families.
- Schedules IFSP meetings with appropriate team members within a 45‑day deadline.
- Schedules ongoing annual assessments by or before the date of the annual FSP.
- Enrolls children within 45 days, and documents family‑driven delays in meeting deadline.
- Develops IFSP containing all essential elements using family‑friendly language.
- Provides Specialized Skills Services, Case Management and other developmental services in the home or other natural environment. Assists in service delivery for other departments when needed, based on licensure or credential.
- Delivers at least 85% of planned services on a monthly basis.
- Keeps no‑shows to a minimum through scheduling regular appointments, keeping program cancellations to a minimum, and communicating the need for keeping appointments to families.
- Uses routine‑based assessment and intervention in developing and implementing the IFSP.
- Conducts service coordination activities for eligible children and their families as per Texas Administrative Code.
- Is knowledgeable about community resources and service delivery systems.
- Addresses transition by the child’s second birthday and adheres to all timelines and transition services.
- Develops transition plans that are individualized.
- Follows all Community Action Corporation of South Texas (CACOST) Policy and Procedures, and ECI Policies and Procedures.
- Observes policies on infection control and universal precautions and uses standard first aid and CPR as needed during the delivery of services.
- Reports accurate statistics as required by monthly direct service logs, case status reports, annual reports, time study, and performance reports.
- Attends weekly supervision and staff meetings as well as scheduled training opportunities.
- Collects and keeps up‑to‑date professional development documentation.
- Registers in the Competency Demonstration System and makes adequate progress by 9 months and 2 years.
- Performs other duties as assigned.
- Two (2) years pediatric experience.
- Three (3) or more years pediatric experience (Preferred).
- A Bachelor’s Degree which includes a minimum of 18 hours of course credit relevant to early childhood intervention.
- Three hours of semester course credit in early childhood development or early childhood special education (Required).
- Valid driver’s license, safe driving record and auto liability insurance (Required).
- Authorization for a criminal history background check and passing a pre‑employment physical exam.
- Case Management experience preferred.
- Proficiency in computer and Microsoft Office programs.
- Case Management experience.
Preliminary physical assessment required before the offer can be accepted. Light to medium physical duties include standing, walking, reaching, bending, hearing, talking, kneeling, floor sitting, pulling, grasping, pushing, stooping, standing, sitting, climbing, occasional running, and the ability to move with agility to carry out IFSP services. Must be able to lift up to thirty‑five (35) pounds. Must be able to work in variable environments related to temperature, humidity, degrees of cleanliness/order, and around other varied environmental factors and elements.
Non‑physical requirements include performing multiple tasks simultaneously, meeting multiple deadlines, making judgment decisions during emergencies, irregular schedules, and working as a team member with other employees and client teams.
